Social Worker

Position: Licensed Clinical Social Worker (LCSW)
Location: Fort Walton Beach, FL
Shift: Full-Time Days with Rotating Weekends
Pay: $24.93–$33.59/hr (based on experience)
Sign-On Bonus: $5,000 (paid in 5 installments over 2 years)
Relocation: Case-by-case support available

Position Overview:
Join a compassionate hospital-based care team serving diverse inpatient units and the emergency department. As a Licensed Clinical Social Worker, you’ll deliver patient-centered support through crisis intervention, discharge planning, biopsychosocial assessments, and emotional care.

Key Responsibilities:

• Perform assessments and psychosocial evaluations for inpatients and ED patients

• Provide crisis intervention, end-of-life support, and discharge planning

• Coordinate care with physicians, RNs, insurance providers, and community partners

• Identify barriers to recovery including financial, psychosocial, or behavioral health needs

• Facilitate family meetings and provide referrals for post-discharge support

• Support high-acuity areas such as NICU, Oncology, Trauma, and ICU

• Uphold Medicare/Medicaid compliance and managed care guidelines

• Engage patients and families with empathy, resourcefulness, and respect for autonomy

Qualifications:

• Master’s in Social Work (MSW)

• Current LCSW licensure (Florida)

• Minimum 2 years of experience (hospital or insurance-based social work preferred)

• Strong communication, multitasking, and critical thinking skills

• Must be organized, proactive, and a self-starter
