Senior Physical Therapist

Join our dedicated healthcare team as a Senior Physical Therapist in Stockton, CA, and play a pivotal role in providing expert rehabilitative care to the elderly and frail population. This position focuses on both clinical and home-based settings, ensuring comprehensive care planning and delivery in collaboration with interdisciplinary teams. The successful candidate will engage in hands-on treatments and supervise therapy programs, aiming to enhance the quality of life and functional abilities of our participants.

Salary: $90,000.00 - $115,000.00 per year

Essential Duties and Responsibilities:

  • Perform thorough Physical Therapy assessments to ascertain care needs and goals for participants both in the facility and at home environments.
  • Engage actively in interdisciplinary team (IDT) meetings to formulate and update participant care plans.
  • Implement and meticulously document Physical Therapy interventions as per physician directives and agreed care plans.
  • Administer various therapeutic exercises and treatments including muscle re-education, gait training, and functional training alongside passive and active exercises.
  • Supervise and train Therapy Assistants to effectively carry out Functional Maintenance Programs.
  • Provide training to staff, participants, and family caregivers on the use and maintenance of therapeutic equipment and techniques.

Minimum Position Qualifications:

  • Bachelor’s Degree in Physical Therapy with a minimum of five years relevant experience.
  • Current licensure as a Physical Therapist in California or proof of eligibility for licensure.
  • Experience in physical therapy within clinical settings, particularly with an elderly or frail population is highly desirable.
  • Demonstrable ability to coordinate with an interdisciplinary team to optimize care plans and outcomes.
  • Excellent documentation and communication skills, essential for team collaboration and compliance with healthcare regulations.

Preferred Qualifications:

  • Master’s Degree or Doctorate in Physical Therapy.
  • Two or more years of experience in a geriatric physical therapy setting.
  • Current CPR certification preferred.
  • Adaptability to changing healthcare environments and participant needs.
  • A patient-focused approach that emphasizes compassionate and culturally sensitive care.
