This role is based in Modesto, CA, where you will work both in our state-of-the-art facility and conduct home visits to ensure comprehensive care. Salary: $90,000 - $115,000 per year
Essential Duties and Responsibilities:
- Conduct detailed Physical Therapy assessments in a clinical setting and in-home environments to identify patient needs, preferences, and objectives.
- Participate actively in interdisciplinary team (IDT) meetings to formulate and update patient care plans.
- Implement and document therapeutic interventions as prescribed by physicians and agreed in care plans.
- Administer manual and mechanical modalities such as massage, heat treatment, hydrotherapy, and electrotherapy.
- Guide and supervise Therapy Assistants to ensure effective delivery of Functional Maintenance Programs (FMP).
- Educate staff, patients, and their caregivers on the use and maintenance of therapeutic equipment and exercises.
Minimum Position Qualifications:
- Bachelor’s Degree in Physical Therapy with a minimum of five years of relevant experience.
- Valid California Physical Therapy License or proof of License eligibility.
- Mastery of physical therapy principles, theories, and methods aimed at maximizing mobility and functionality.
- Experience in a clinical setting providing therapy to a frail or elderly population.
- Knowledge of and compliance with California healthcare regulations.
Preferred Qualifications:
- Master’s Degree in Physical Therapy or Doctorate in Physical Therapy.
- Two or more years of direct experience working with the elderly in diverse residential settings.
- Previous supervisory or leadership experience in a clinical setting.
- Active CPR certification.
- Demonstrated ability to train and lead a team in complex patient care activities.
