Role Overview
Job Description :
  • This is a Nurse Educator role focused on coordinating a Diabetes Education Program within an outpatient clinic setting.
  • You will lead patient education initiatives while overseeing clinical staff and ensuring high-quality diabetes care and program delivery.
  • Coordinate and manage the Diabetes Education Program in an outpatient setting.
  • Provide education to patients on diabetes management, treatment plans, and lifestyle modifications.
  • Supervise a team including 2 RNs and 1 Dietician.
  • Monitor patient progress and ensure effective care outcomes.
  • Perform administrative duties including documentation, reporting, and program oversight.
  • Collaborate with providers and interdisciplinary teams to optimize patient care.
Mandatory Skills/Experience :
  • Must have Active RN license (CT or compact).
  • Must have BLS.
  • Must have Certification as a Certified Diabetes Care and Education Specialist (CDCES) and/or Board Certified in Advanced Diabetes Management (BC-ADM).
  • Must have Strong background in diabetes education and outpatient care.
  • Must have Previous travel experience in educator or leadership roles
