Essential Duties and Responsibilities:

• Conduct in-home visits to meet with clients and their families, establishing a rapport and a comprehensive understanding of their needs.
• Create detailed and tailored plans of care that encompass both nursing and supportive services to fully address the client's requirements.
• Select and manage a dedicated care giver team whose skills match the individual needs of each client, ensuring optimal care delivery.
• Maintain daily availability for clients and the caregiving team to supervise and ensure the delivery of services aligns precisely with client preferences and schedules.

Minimum Position Qualifications:

• Must have received the COVID vaccination.
• Holds a valid nursing license; BSN preferred.
• Proven supervisory experience in a healthcare setting.
• Essential prior home care experience.
• A minimum of 3 years of clinical experience in nursing.
• A valid US driver's license.

• Experience in managing private pay aspects in home care scenarios.
