About the Position

The Skilled Nursing Facility RN provides comprehensive nursing care to residents utilizing the nursing process, including assessment, care planning, implementation, evaluation and documentation.

You'll work closely with physicians, CNAs, rehabilitation therapists and other members of the interdisciplinary healthcare team to provide safe, compassionate and individualized care.

This is an excellent fit for nurses coming from Skilled Nursing, Long-Term Care, Rehabilitation, Med-Surg, Acute Care or other hospital-based nursing environments.

Registered Nurse Responsibilities

• Perform comprehensive resident assessments and monitor changes in condition
• Develop, implement and update individualized nursing care plans
• Administer medications, treatments and therapies safely and accurately
• Coordinate admissions, discharges and transfers
• Participate in interdisciplinary care conferences
• Collaborate with physicians, CNAs, therapists and other healthcare professionals
• Provide resident and family education
• Maintain accurate and timely EMR documentation
• Support infection prevention, patient safety and quality initiatives
• Promote resident dignity, independence and quality of life
• Provide compassionate, patient-centered nursing care

RN Qualifications

• Current California Registered Nurse (RN) license, or ability to obtain California licensure by endorsement
• Graduate of an accredited nursing program
• Current BLS/CPR certification
• Minimum one year of nursing experience preferred
• SNF, LTC, Med-Surg, Rehab, Acute Care or Hospital RN experience highly valued
• Strong clinical assessment and critical-thinking skills
• Excellent communication and interdisciplinary teamwork skills

Compensation & Relocation Incentives

$7,500 Sign-On Bonus

Night Shift: Additional $10/hour differential

For qualifying candidates relocating from more than 35 miles away:

$1,500/month housing stipend for the first three months, plus relocation assistance.
