Psychiatric Mental Health Nurse Practitioner
Job description
Conduct patient visits in a clinic setting
• Diagnose patient conditions
• Medication management and psychotherapy
• Access patient information
• Assess existing psychiatric disorders; diagnose and formulate plan of care, write
referrals, renew medications, order diagnostics and manage results.
• Clinical duties will include psychiatric and mental health evaluations, complete
assessments on clients, treatment plans, and Medication-Assisted Treatment for
SUD.
• Support the patient’s management of chronic diseases through management of
their behavioral health conditions.
• Plan of Care and Treatment: Plans and implements therapeutic interventions to
assist the patient to achieve an optimal state of health.
• Maintains productivity and documentation standards, and engagement in EHR
system testing and audit preparation efforts.
• Cultivates strong patient relationships.
• Creates medical records, collects patients' clinical data, and documents patients
encounters.
** MUST HAVE CURRENT PMHNP AZ LICENSE IN GOOD STANDING.
