DESCRIPTION:
( In-person client appointments, collaboration with care team, contact with guardian's about medication changes, documentation, etc.)
Duties
General Responsibilities:
- Conduct physical exams, order and interpret lab tests, and make diagnoses.
- Develop and manage treatment plans for common medical conditions (chronic and acute).
- Provide preventative care and health education.
- Prescribe medications, order appropriate diagnostic tests, and manage follow-up care.
- Assess, diagnose, and treat mental health conditions.
- Prescribe psychiatric medication and manage psychiatric treatment plans.
- Collaborate with other healthcare professionals to ensure patient care.
Hybrid Work Responsibilities
- In-office work may include physical exams, patient assessments, and follow-up visits.
- Remote work may involve telehealth consultations, charting, patient care coordination, and documentation.
Expected Deliverables
- Accurate and timely documentation of patient assessments and treatment plans.
- Effective patient and guardian communication via telehealth, phone, and in-person visits.
- Compliance with regulatory standards for both psychiatric and primary care.
Residential Treatment Program Responsibilities

Intake Evaluations and Documentation
-
Initial Psychiatric Evaluations: The NP will conduct a thorough psychiatric evaluation within 48 hours of client intake to determine the immediate mental health needs of residential clients.

History and Physical (H&P): The NP will complete and/or review and sign off on a history & physical for each client within 48 hours of intake, ensuring both psychiatric and general health considerations are addressed.

Ongoing Psychiatric Care:

Weekly Psychiatric Follow Ups: the NP will provide weekly psychiatric follow-up visits for each residential client to monitor progress, medication management, and adjust treatment plans as necessary.

Medication Management: the NP will manage and adjust psychiatric medications, ensuring the clients receive timely prescriptions and appropriate dosages.

Communications: the NP will maintain weekly and as needed communication with clients and guardians to update them on the progress of treatment, including medication adjustments and any provider recommendations for further care.
Partial Hospitalization Program (PHP) Responsibilities:

Initial Evaluations and Documentation
-
The NP will conduct initial evaluations for clients who do not have an existing outpatient psychiatric provider to ensure continuity of care and timely intervention.

Ongoing Psychiatric Care

Monthly Follow Ups: the NP will provide monthly and as needed psychiatric follow ups for clients requiring medication management and ongoing psychiatric care.

Medication Management: the NP will ensure appropriate medication management for clients in PHP, adjusting as needed based on clinical reports and observations.

Multidisciplinary Collaboration

The NP will attend weekly multidisciplinary staffing meetings for each PHP program, working with the team to evaluate client progress and adjust treatment plans as necessary.
Education and Licensure
- Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP) by an accredited certification body
- Psychiatric Mental Health Nurse Practitioner (PMHNP) by an accredited certification body
- Arizona state licensure (if different from certification)
- DEA and other required certifications for medication prescribing.
Experience
- Preferred 2+ years of experience (preferably in both primary and mental health settings).
- Strong history of patient-centered care, critical thinking, and collaborative practice.
Skills
- Excellent clinical assessment and diagnostic skills.
- Ability to work effectively with patients across a variety of age groups.
- Proficient in using Electronic Health Records (EHR).
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Ability to manage multiple responsibilities and patients across both FNP and PMHNP duties.
