We are currently hiring a Program Manager to supervise, organize and coordinate supports for individuals in their own homes, in natural family homes and/or Lifesharing and/or in community-based licensed residential programs. This region would be for Lackawanna, Luzerne, and Wyoming counties. The Program Manager could oversee up to 5 homes or programs in a specific territory with travel reimbursement.

Responsibilities

  • Responsible for comprehensive understanding of regulations.
  • Ensures all programs assigned are in compliance with PA State Code, Title 55, Department of Public Welfare, CLA Programs Chapter 6400 regulations, Family Living Services Chapter 6500 regulations, and/or Unlicensed In Home & Community Support locations (compliant to Chapter 51 regulations) as well as in compliance with Agency policies and procedures.
  • Participates and/or develops, if applicable, as the Plan Lead in the development of the Individual Service Plan, ISP Annual Update and all ISP revisions (fulfilling the role of Program Specialist).
  • Attends the ISP, ISP Annual Update and all ISP revisions.
  • Responsible for the coordination/completion of individual assessments for the development of the ISP, ISP Annual Update and all ISP revisions.
  • Review the ISP, Annual ISP Update and all ISP revisions.
  • Reviews all referrals by the county MH/ID program along with the Program Director and makes recommendations regarding individual admission to the program.

  • Direct supervision of Direct Support Staff Supervisors or Lifesharing Direct Support Staff with regularly scheduled face-to-face contact.
  • Completes Performance Evaluations for Direct Support Staff Supervisors or Lifesharing Direct Support Staff and reviews Performance Evaluations completed by Direct Support Staff Supervisors.
  • Participates in facilitating supervisors’ meetings, inclusive of generating meeting agenda, directing the meeting, assigning tasks, and eliciting staff participation.

  • Review records of Direct Support Staff Supervisor’s supervision with employees.
  • Assists Direct Support Staff Supervisors with disciplinary actions.
  • Conducts unannounced program monitoring visits on a regular basis to ensure areas of health, safety, and consumer welfare are compliant with Agency policies and procedures as well as regulatory requirements
  • Facilitates the completion of orientation training for all new employees.
  • Ensures staffing patterns reflect optimum opportunity for consumers to experience quality care and community integration.
  • Ensures incidents are filed electronically in the PA EIM system
  • Conduct quality investigations and complete quality investigation reports.
  • Ensures the completion and documentation of monthly site inspections to ensure cleanliness and safety and monitors sites through direct observation of various hours of service

Minimum Qualification & Requirements

  • An associate’s degree or 60 credit hours from an accredited college or university plus 4 years’ experience working with individuals with developmental disabilities
  • Must possess and maintain a current, valid driver’s license from state of residency.
