Schedule: Full-Time Monday–Friday 8:00 AM – 5:00 PM (No weekends or on-call)
Compensation: $230K–$245K base + up to $20K in performance bonuses

Your Role:

• Deliver proactive, preventative care to Medicare-eligible patients

• Manage a consistent panel of 15–18 patients daily

• Collaborate with care teams and leadership on health outcomes

• Use Canopy and Greenway EMRs for streamlined documentation

• Lead team performance aligned with patient-centered goals

What You’ll Need:

• MD or DO, Board Certified/Eligible in FM or IM

• Active GA medical license + DEA

• U.S. work authorization

• Geriatric experience or value-based care background preferred
