Physical Therapist (on-site)

Our client is a healthcare provider dedicated to delivering exceptional care to a rural community in central WI. While situated in a smaller city, the team offers a close-knit and supportive environment with access to post-acute home health and hospital-based services. As a Physical Therapist, you will provide personalized, high-quality care in both outpatient and home health settings, impacting the lives of a diverse range of patients.

Location: Rural healthcare setting with proximity to larger cities.

Schedule: Day shifts, full-time 40 hours per week, with a weekend rotation at the hospital required only once every 10 weeks.

Focus:

  • Provide physical therapy treatments to patients in both the hospital outpatient setting and post-acute home health.
  • Develop personalized therapy plans to optimize patient recovery, mobility, and quality of life.
  • Educate patients and caregivers on continued care and safe physical practices.
  • Collaborate with a healthcare team dedicated to providing compassionate, effective treatment in a rural setting.
  • Maintain thorough documentation of patient progress, adhering to regulatory standards for both hospital and home health environments.

Skill Set:

  • Active Physical Therapist licensure in Wisconsin or the ability to obtain a license before the start date.
  • Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
  • Strong communication skills and the ability to build rapport with patients and families in diverse clinical settings.
  • Efficient time-management and adaptability to care for varying caseloads in both the hospital and home health.
  • Detail-oriented, with excellent organizational skills for documentation and treatment planning.
