Physical Therapist

Essential Duties and Responsibilities:

  • Conduct comprehensive physical assessments and review medical histories to establish patient conditions and needs.
  • Create and oversee personalized treatment plans aimed at meeting individual rehabilitation goals and enhancing quality of life.
  • Implement therapeutic exercises, functional training, and other rehabilitative activities to improve patients' physical abilities and independence.
  • Provide education to patients and their families on wellness, injury prevention, and the recovery process, empowering them towards better health management.
  • Maintain detailed and accurate records of patient's progress, adapting treatment plans as necessary to achieve optimal outcomes.
  • Work collaboratively with other health care professionals, ensuring a coordinated approach to patient care.

Minimum Position Qualifications:

  • Graduation from an accredited Physical Therapy program.
  • Minimum of nine months of experience in a physical therapy role, preferably with some experience in home health settings.
  • Strong interpersonal and communication skills, capable of effectively interacting with patients, families, and caregivers.
  • Active Physical Therapist license or eligibility for licensure in the state of employment.
  • Demonstrated ability to work autonomously and manage a significant patient caseload effectively.

Preferred Qualifications:

  • Experience with electronic health record (EHR) systems and proficiency in documenting and tracking patient progress.
  • Skills in utilizing physical therapy modalities and equipment with a high degree of technical adeptness.
