Responsibilities:

• Provide comprehensive physical therapy services including patient assessments, treatment planning, therapeutic exercises, manual therapy, and patient education.

• Deliver ergonomic assessments and injury prevention training on-site.

• Collaborate effectively within an interdisciplinary medical team (physicians, nurses, occupational therapists, medical assistants, and health coaches).

• Maintain detailed, compliant documentation in electronic medical records.

• Educate patients in-home exercise programs and self-care techniques.

• Assist in care coordination, facilitating appropriate referrals and follow-ups.

• Develop and conduct educational sessions for staff on injury prevention and ergonomics.

• Participate in ongoing professional development and compliance training.

Requirements:

• BS, MS, or Doctorate in Physical Therapy from a CAPTE-accredited institution.

• Active Physical Therapist license in Tennessee.

• Minimum of 3 years’ clinical experience, preferably outpatient setting with adolescents and adults.

• Current Basic Life Support (BLS) Certification.

• Proficiency in electronic medical records documentation.

• Experience or certification in ergonomics is strongly preferred.
