Physical Therapist - Skilled Nursing Facility

Salary: $93,600.00 - $114,400.00 per year

Essential Duties and Responsibilities:

  • Evaluate residents' conditions and develop personalized rehabilitation plans.
  • Implement physical therapy interventions to improve mobility and reduce pain.
  • Guide residents in the use of therapy equipment and exercise protocols.
  • Provide hands-on therapy and recommend home exercise programs.
  • Document resident progress and adjust treatments as necessary.
  • Ensure adherence to facility safety protocols and infection control measures.
  • Supervise and collaborate with physical therapy assistants and other healthcare staff.
  • Maintain resident confidentiality and uphold facility standards.
  • Participate actively in interdisciplinary team meetings to optimize patient care.

Minimum Position Qualifications:

  • Graduate of an accredited Physical Therapy Program.
  • Current New York State PT License.
  • Ability to communicate effectively with residents and their families.
  • Strong attention to detail and accuracy in documentation.
  • Basic computer literacy and proficiency in clinical software.

Preferred Qualifications:

  • Experience in a long-term care or skilled nursing facility.
  • Additional certification in physical therapy specialties.
  • Prior exposure to electronic medical records systems.
  • Proven ability to handle complex cases and multi-task in a fast-paced environment.
