Salary: $93,600.00 - $114,400.00 per year
Essential Duties and Responsibilities:
- Evaluate residents' conditions and develop personalized rehabilitation plans.
- Implement physical therapy interventions to improve mobility and reduce pain.
- Guide residents in the use of therapy equipment and exercise protocols.
- Provide hands-on therapy and recommend home exercise programs.
- Document resident progress and adjust treatments as necessary.
- Ensure adherence to facility safety protocols and infection control measures.
- Supervise and collaborate with physical therapy assistants and other healthcare staff.
- Maintain resident confidentiality and uphold facility standards.
- Participate actively in interdisciplinary team meetings to optimize patient care.
Minimum Position Qualifications:
- Graduate of an accredited Physical Therapy Program.
- Current New York State PT License.
- Ability to communicate effectively with residents and their families.
- Strong attention to detail and accuracy in documentation.
- Basic computer literacy and proficiency in clinical software.
Preferred Qualifications:
- Experience in a long-term care or skilled nursing facility.
- Additional certification in physical therapy specialties.
- Prior exposure to electronic medical records systems.
- Proven ability to handle complex cases and multi-task in a fast-paced environment.
- Demonstrated leadership in managing patient care teams.
