Occupational/Physical Therapist
Northern, Arizona

Join a 109-bed skilled nursing and long-term care facility located in Northern, Arizona in the heart of Arizona in the middle of Tonto National Forest. We have a secure memory care and behavioral health unit, long term care and rehabilitation units.


  • Help patients with daily living skills and self-care tasks (i.e., "occupations") such as getting dressed
  • Support patients with memory loss or other cognitive issues
  • Make recommendations about adaptive equipment
  • Advise pertinent parties about patients' accessibility needs
  • Documents according to professional practice guidelines set by regulatory agencies. Documents legibly.
  • Completes documentation within the established time frames of the department.
  • Documents evidence based and individualized treatment techniques/plan.
  • Documentation reflects skilled intervention and comprehensive assessment of the patient's progress or lack of progress.
  • Treatment goals are reassessed according to department policies.
  • Communicates pertinent patient information to ensure the best possible follow through of care.
  • Attends department required meetings and/or follows up to obtain pertinent information.
  • Participates in departmental/facility process improvement.
  • Meets productivity standards set by the department.


  • Master’s Degree in Occupational Therapy
  • AZ Physical Therapist License
  • New Graduates- OK
  • Myofascial Release Certification
  • Excellent communication skills (interpersonal, written, verbal)
  • Strong attention to detail
  • Excellent time management skills
  • Excellent computer skills. Prior use of electronic health record preferred.
  • Outgoing and energetic personality
  • Ability to multitask, prioritize projects as needed, and pay strong attention to detail
