About the Position:
Reporting to the I-SNP Team Lead, the Nurse Practitioner will independently manage a caseload of 90-100 patients across multiple nursing and adult home facilities, focusing on clinical management and proactive care to prevent hospitalizations. This role emphasizes treating in place, collaborating with social workers and RNs to deliver comprehensive care. The NP sets their own schedule, conducting routine visits, acute assessments, and post-hospitalization follow-ups while fostering strong relationships with facility staff. The position follows a Monday-Friday, 8-5 schedule, with some on-call responsibilities as needed. FCNY offers a team-oriented culture with strong retention and career longevity.
Duties & Responsibilities:
Patient Care & Clinical Management
- Manage a caseload of 90-100 patients across multiple nursing and adult home facilities.
- Conduct initial assessments, routine visits, and acute care evaluations.
- Perform post-hospitalization follow-ups within two days of discharge.
- Monitor and treat wounds with weekly evaluations as needed.
Interdisciplinary Collaboration
- Work closely with social workers, RNs, facility staff, and PCPs to coordinate care.
- Participate in care planning and communicate changes in patient health to the medical team.
- Foster strong relationships with assigned facilities and maintain open communication with staff.
Scheduling & Autonomy
- Set and manage own daily patient visit schedule based on clinical priorities.
- Ensure timely documentation of patient encounters in the MedEnt EHR system.
On-Call & Additional Responsibilities
- Participate in a rotation for some on-call coverage as needed, including weeknight and weekend shifts.
- Provide backup coverage when needed, with additional compensation for unscheduled calls.
- Maintain compliance with facility credentialing requirements and FCNY quality standards.
Performance & Professional Development
- Meet quality and utilization goals tied to performance-based incentives.
- Engage in continuous learning with a minimum of 35 CMEs per year.
Qualifications:
Required
- Active Nurse Practitioner (NP) license in New York State with board certification.
- Strong clinical judgment and ability to identify subtle changes in patient health.
- Effective written and verbal communication skills for working with patients, caregivers, and facility staff.
- Basic computer skills and proficiency in electronic health records (EHR) (MedEnt experience a plus).
- Ability to work independently while maintaining strong facility relationships.
- Maintain active NYS licensure, board certification, CPR certification, and provide own vehicle with auto insurance for travel to facilities.
Preferred
- Experience in geriatric nursing and managing care for frail individuals.
- Familiarity with managed care systems and interdisciplinary team collaboration.
Compensation:
The Nurse Practitioner position offers a starting salary range of $120,000-$130,000, with a $10,000 sign-on bonus and up to $10,000 annual bonus. On-call responsibilities are compensated through additional PTO and additional pay for unscheduled coverage. First-year total compensation typically ranges from $140K-$160K, with opportunities for further earnings based on performance and additional patient visits. Time off includes 4 weeks PTO, 48 hours of sick pay, 3 days for CMEs, and 2 floating holidays.

CONTACT RESPONSIBILITY:
INTERNAL: COO of FCNY, other Advanced Practice Providers (APP’s), FCNY Medical Directors, UM staff on beneficiaries utilization of care authorizations; SW staff on psychosocial issues or Advance Directive; Administrative Staff for credentialing, payroll and other office matters.
EXTERNAL: Beneficiaries and their representatives, PCPs, Consultants and facility staff personnel to review and discuss matters pertaining to the care of the beneficiary. Other health care professionals when serving on committees or researching information.
POSITION SCHEDULE REQUIREMENTS
The NP is responsible for coverage (including in person and telephonic) of assigned facilities and/or covering facilities from 8 am-5 pm, Monday through Friday. Periodically, this will also require coverage when the weekday on-call provider has time off, be it scheduled or unscheduled, from 5pm – 8am. The NP will periodically provide weekend coverage during assigned weekends from Friday 5 pm-Sunday 5 pm.
PHYSICAL REQUIREMENTS:
- Ability to speak clearly, see and hear individuals.
- Ability to see beneficiaries within facilities.
- Ability to attend meetings, perform analytical desk work and travel throughout the Counties that Family Choice serves.
- Ability to operate motor vehicle to travel to facilities, beneficiaries’ homes within the community or related locations.

- Ability to work extended hours as necessary.
ENVIRONMENTAL CONDITIONS:
Work is performed at either a medical facility or beneficiary's residence. Normally there is appropriate lighting, ventilation and good working conditions. Some exposure to medical devices and related equipment. When traveling outside the facility, subject to normal travel conditions (weather, traffic, etc.).
DETAILED DUTIES AND RESPONSIBILITIES – ESSENTIAL FUNCTIONS:
The primary role of the Nurse Practitioner (NP) is to manage care for an assigned caseload of enrollee’s with the enrollees PCP as well as the entire interdisciplinary care team (ICT). This includes physical assessments, contact with the Utilization Management department, entering notes into an Electronic Health Record and recording data on quality indicators while adhering to the CMS/State guidelines pertaining to institutional care and services for the frail and elderly populations.
TASKS:
- Conduct an initial assessment of new enrollee’s by reviewing case files, meeting with the beneficiaries, their representatives, and facility staff. This collaborative effort includes obtaining information from physicians or other medical personnel.
- Meet and maintain partner facility’s specific credentialing requirements.
- Visit each assigned enrollee on a routine monthly/bi-monthly basis or more frequently based on acuity according to the enrollee’s vulnerability (for example those with recent hospitalization and those with skin issues should be seen weekly). Visits may be at the nursing/adult home facility, or other venue.
- Review and record the results of each visit and/or other required documentation for each beneficiary in EHR within specified timeframes of evaluation, unless unavoidable circumstances are noted.
- Discuss and document any problems or concerns of the enrollee with them and/or other members of the ICT.
- Identify changes in health and report to the appropriate medical personnel.
- Contact and maintain communication with enrollee or representative on routine or more frequent basis based on FCNY guidelines.
- Communicate regularly with the enrollee’s PCP based on the PCP’s preferences as documented in the Physician Agreement and as specified in the Model of Care. This includes documentation in the enrollee’s electronic record per company guidelines.
- Cover other facilities at the request of management.
- Communication of enrollee’s acuity/status to UM department when noted or when outreach by UM staff occurs.
- Complete all required visits, forms and documentation in a timely manner per guidelines and company policy.
- Attend required meetings as identified by the Clinical Team Management Staff, Medical Director and/or COO of FCNY.
- Goals of Care discussions with enrollee’s and/or representative which may include DNR, Health Care Proxy, MOLST
- Quality Documentation that reflects clinical picture of enrollee’s including past medical history and accurate diagnoses ensuring that ICD10 codes are being utilized and are fully supported in the documentation.
- Other duties as assigned.
DETAILED DUTIES AND RESPONSIBILITIES - NON-ESSENTIAL FUNCTIONS:
- Conduct informal facility staff education and teaching to improve the effectiveness and efficiency of care delivery if and when requested by Clinical/Operations team.
- Perform all related duties, as assigned.
This position shall require, at various times and to varying degrees, unlimited access to and use of all categories of protected health information (“PHI”), as such term is defined in HIPAA, to enable the employee to fully and competently provide the full range of services required by this position. Notwithstanding the foregoing, the employee filling this position shall make reasonable efforts to limit the employee’s use and disclosure of PHI to the minimum amount necessary to accomplish the intended purpose of the specific use or disclosure of the PHI, in accordance with the company’s policies and procedures.
