Nurse Practitioner or Physician Assistant – Skilled Nursing Facility

Provide hands-on, patient-centered care in a skilled nursing facility in The Bronx. You'll join a collaborative team focused on improving quality of life for both short- and long-term residents through compassionate, evidence-based care.

What You’ll Be Doing

• Serve as primary care provider for skilled nursing residents
• Perform physical exams, assessments, and medical history reviews
• Develop and implement patient-centered care plans
• Collaborate with physicians and interdisciplinary staff
• Educate residents and families on treatment plans

About You

• Be able to do the job as described
• Board Certified Nurse Practitioner or Physician Assistant
• Ability to work in a skilled nursing or post-acute setting
• Strong communication skills with patients, families, and care teams
• NYS NP or PA-C license
