We are seeking a dedicated Nurse Practitioner to join our nursing home team in Waterloo, NY. This full-time, direct hire position is tailored for a compassionate practitioner experienced in adult or family care, particularly comfortable working in a long-term care setting. Salary: $115,000-$135,000 per year
Essential Duties and Responsibilities:
- Provide comprehensive patient care and establish a long-term care plan for adults in a nursing home setting.
- Collaborate with healthcare professionals to monitor the health conditions of patients and adjust treatments as necessary.
- Conduct routine check-ups on patients to assess their health condition and discover possible issues.
- Give appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall health.
- Coordinating with onsite child day care services as part of family care initiatives.
- Participate in the analysis, development, and implementation of strategic healthcare service improvements.
- Oversee medical interventions administered by team members and other practitioners.
Minimum Position Qualifications:
- Must have an active NP Certification.
- Possession of a NYS Nurse Practitioner License.
- Graduate from an accredited Nurse Practitioner or Physician Assistant program.
- Proven experience in geriatric, adult, or family practice, preferably in long-term care environments.
- Strong interpersonal and communication skills.
- Ability to work autonomously and in a team environment.
Preferred Qualifications:
- Experience with patient care in long-term residential settings.
- Familiarity or certification in palliative care practices.
- Demonstrable leadership or management experience in a healthcare setting.
- Experience in handling chronic illnesses common among elderly patients.
- Active participation in community-based health initiatives.
- Advanced training in diagnostics and patient care management systems.
