Nurse Practitioner - Long Term Care Facility

We are seeking a dedicated Nurse Practitioner to join our nursing home team in Waterloo, NY. This full-time, direct hire position is tailored for a compassionate practitioner experienced in adult or family care, particularly comfortable working in a long-term care setting. Salary: $115,000-$135,000 per year

Essential Duties and Responsibilities:

  • Provide comprehensive patient care and establish a long-term care plan for adults in a nursing home setting.
  • Collaborate with healthcare professionals to monitor the health conditions of patients and adjust treatments as necessary.
  • Conduct routine check-ups on patients to assess their health condition and discover possible issues.
  • Give appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall health.
  • Coordinating with onsite child day care services as part of family care initiatives.
  • Participate in the analysis, development, and implementation of strategic healthcare service improvements.
  • Oversee medical interventions administered by team members and other practitioners.

Minimum Position Qualifications:

  • Must have an active NP Certification.
  • Possession of a NYS Nurse Practitioner License.
  • Graduate from an accredited Nurse Practitioner or Physician Assistant program.
  • Proven experience in geriatric, adult, or family practice, preferably in long-term care environments.
  • Strong interpersonal and communication skills.
  • Ability to work autonomously and in a team environment.

Preferred Qualifications:

  • Experience with patient care in long-term residential settings.
  • Familiarity or certification in palliative care practices.
  • Demonstrable leadership or management experience in a healthcare setting.
  • Experience in handling chronic illnesses common among elderly patients.
  • Active participation in community-based health initiatives.
  • Advanced training in diagnostics and patient care management systems.
