Nurse Practitioner - Long-Term Care Facility

Salary: $115,000 - $130,000 per year

Essential Duties and Responsibilities:

  • Conduct comprehensive assessments of residents' health statuses, identifying acute and chronic conditions, and crafting personalized care plans.
  • Oversee medication management including prescription, administration, and monitoring of efficacy and side effects.
  • Manage common chronic conditions prevalent among the elderly such as diabetes, hypertension, heart disease, and dementia.
  • Provide immediate care for acute illnesses and injuries, ensuring prompt and thorough response to resident emergencies.
  • Collaborate with an interdisciplinary team including physicians, nurses, therapists, and social workers to optimize patient outcomes.
  • Communicate effectively with residents and their families, conveying treatment plans and health advice clearly and compassionately.
  • Maintain meticulous medical records, documenting all patient encounters and interventions inline with legal and medical standards.
  • Engage in ongoing quality improvement initiatives focused on enhancing care delivery and patient safety.

Minimum Position Qualifications:

  • NP Certification required.
  • Must be licensed or eligible for licensing in New York State.
  • Proven experience as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) or closely related specialty.
  • Strong foundation in the diagnosis and management of complex patient cases.
  • Capable of critical decision-making skills and quick problem-solving abilities.
  • Exceptional organizational and time-management skills.

Preferred Qualifications:

  • Board Certification in a relevant nurse practitioner specialty.
  • Experience in a long-term care or geriatric medical facility.
