About You

• Be able to do the job as described
• Experienced Nurse Practitioner seeing patients in office or home setting (no new grads)
• Some experience with hospice or palliative care is a plus
• Active Nurse Practitioner license in North Carolina (or ability to obtain)
• Strong ability to work independently in a home care setting

What You’ll Be Doing

• Provide in-home hospice and palliative care to patients
• Conduct patient assessments and develop care plans focused on comfort
• Support and educate patients and families on end-of-life care and advanced directives
• Collaborate with physicians, consultants, and interdisciplinary care teams
• Ensure delivery of high-quality, compassionate hospice services
