Nurse Manager - Hospital Readmissions (RN)

Currently seeking a talented and dedicated Hospital Readmissions Manager Registered Nurse RN for a great opportunity in the Central NC area. The ideal candidate will possess 5+ years of current experience working in acute care hospitals in the and 3+ years of experience working experience in Case Management, Readmission, or Quality in a leadership role.

Responsibilities of the Nurse Manager Hospital Readmissions RN

  • The Manager of Hospital Readmissions coordinates care between acute and post-acute providers by empowering and encouraging patients to be involved in their own health care, therefore, facilitating patient accountability in the safe transition of care and ensuring positive quality outcomes, with a focus on reducing unnecessary hospital readmissions
  • Maintains a staff of qualified social workers and nursing personnel as budgeted to provide case management/utilization management for his/her assigned patient population.
  • Maintains a staff of qualified nurses and qualified personnel to provide transitional care services to discharged patients
  • Maintains adequate facilities, supplies, and equipment consistent with meeting the areas of responsibility delegated to him/her
  • Establishes efficient operating management throughout the areas of responsibility

Requirements of the Nurse Manager Hospital Readmissions RN

  • Bachelor of Science in Nursing
  • Master's of Science in Nursing is preferred
  • Current license to practice as a Registered Nurse in North Carolina
  • 5 years of experience as a Registered Nurse, preferably experience in Case Management, Readmission, or Quality
  • Knowledge of regulatory agencies
  • Extensive computer knowledge is required
  • Excellent analytical skills to compile and present data to professionals with recommendations for improvement
  • Strong interpersonal and communication skills
