Our client is a multi-site home health company with locations in NY and MA.  They have become a national leader in providing affordable, innovative healthcare for Dual-eligible individuals.

They have many openings for Case Managers that are Bi-Lingual located in the NY Boroughs.

The Nurse Care Manager, Community works in collaboration with the member’s Primary Care Provider and an interdisciplinary team to proactively manage members with complex medical, behavioral and psychosocial needs. The Nurse Care Manager, Community coordinates and facilitates development, implementation and monitoring of Individual Care Plans (ICP)/Person Centered Service Plan (PCSP) based on a member’s goals, preferences and disease states.  The goal of the Community Care Manager is to link members to appropriate care, services, and community resources, promote self-management in order to improve the health status, functioning, community integration and quality of life. 

Requirements:

·        Must be Bilingual (English/Spanish)

·        Must have UAS Experience

·        RN License in the State of NY

·        2+ year experience in Home Health or related field

·        Knowledge of Medicare and Medicaid

·        Up To 50% travel to client homes
