This position will report to the Director and have over 170 FTEs reporting to it!
Position Summary
Manages and coordinates clinical and operational functions of the IP Care Management department. Oversees the efficiency and effectiveness of the clinical care management team, made up of nurse care managers, care management coordinators and social workers. Addresses customer concerns/complaints related to all designated clinical areas. Assists in the development and oversight of the department quality monitoring and educational plans. Provides guidance and oversight of all change, pertinent change management activity. Oversees performance evaluations for clinical staff. Collaborates with the leadership team to develop departmental goals and provide input related to the development and management of the budget. Ensures compliance and regulatory requirements addressed and maintained in department practices as well as policies and procedures. Serves as liaison to state and local educational institutions, approving and coordinating all student internships within the department.
Qualifications
Basic Qualifications:
Education:
- Bachelor's Degree or equivalent in Nursing; OR Master's degree in Social Work or Health Care related field
- 5 years of experience typically gained through skills/knowledge/abilities in relevant clinical experience
- 1 year of experience typically gained through skills/knowledge/abilities in supervisory experience or equivalent combination of education, training, and years of experience
- Experience working in an organization of size and complexity comparable to Health services
Preferred Qualifications
- MSN is strongly preferred
- RN license is strongly preferred
- Prefer 3-5 years of care management leadership experience; manager level or above
- Care Management certificate a plus
- Prefer knowledge and experience collaborating across all care management settings, including inpatient, post-acute care and ambulatory care management settings
- Prefer knowledge and experience across various patient populations; ED settings, adult and women and children's patient populations
