What You’ll Do
MDS Assessment & Care Planning
Coordinate the interdisciplinary MDS assessment process
Ensure timely and accurate completion and transmission of all MDS assessments
Develop and support comprehensive resident care plans
Regulatory Compliance & Documentation
Maintain compliance with federal and state regulations as well as Medicare guidelines
Ensure documentation accuracy and completeness
Support reimbursement integrity through accurate clinical assessment
Clinical Coordination
Collaborate with nursing, therapy, and interdisciplinary teams
Monitor resident condition changes that may affect assessment timelines
Support clinical teams in maintaining strong documentation standards
Quality Improvement
Monitor assessment deadlines and regulatory timelines
Identify opportunities to improve documentation accuracy and care outcomes
Assist leadership in maintaining high-quality clinical standards
Who the Organization Is Seeking
You Must Bring
✓ Active RN License – Unencumbered Registered Nurse license
✓ MDS Experience – Minimum one year of recent MDS completion experience
✓ Clinical Knowledge – Strong understanding of skilled nursing regulations and care planning
✓ Organizational Skills – Ability to manage multiple deadlines and documentation requirements

Competitive Compensation
$75,000 – $99,000 annually depending on experience
Consistent Schedule
Full-time, Monday – Friday
8:00 AM – 4:30 PM
On-site position with participation in on-call rotation
