A well-established skilled nursing facility is seeking a Registered Nurse (RN) MDS Coordinator to lead the assessment and reimbursement process. This role is ideal for a detail-oriented clinician who understands the impact of MDS on quality measures, reimbursement, and resident care planning.

Responsibilities

  • Oversee and complete all MDS/RAI assessments (MDS 3.0) in compliance with CMS guidelines

  • Ensure timely and accurate CMS submissions with minimal error rates

  • Collaborate with interdisciplinary teams (therapy, nursing, dietary, social services) to gather accurate assessment data

  • Translate MDS findings into comprehensive, actionable care plans

  • Monitor and optimize PDPM performance, CMI scores, and reimbursement accuracy

  • Maintain compliance with Title 22 and CMS regulations

  • Utilize clinical software (PointClickCare preferred) and support team training as needed

  • Participate in QA/QAPI initiatives and support survey readiness

Qualifications

  • Active California RN license in good standing

  • Minimum 3+ years of MDS completion experience in a skilled nursing or subacute setting

  • Strong proficiency in RAI/MDS 3.0 processes

  • Working knowledge of California Title 22 and CMS regulations

  • Experience with clinical software systems (PointClickCare preferred)

  • Demonstrated ability to meet submission deadlines with high accuracy

  • Excellent communication and interdisciplinary collaboration skills

  • Strong critical thinking, organization, and attention to detail

Preferred

  • RAC-CT Certification (or actively pursuing)

  • Experience with PointRight or similar MDS scrubber tools

  • Deep understanding of PDPM, Five-Star Quality Measures, and reimbursement strategies
