Licensed Practical Nurse (LPN)

This position ensures the organization can fulfill its mission of providing medically necessary, evidence based integrated health care to patients. LPNs ensures the organization responds to medical needs, provides necessary continuity of care services for those patients with chronic care conditions, and provides appropriate health care services for needs that emerge during Patients' stay. The role of the Licensed Practical Nurse (LPN) is primarily administration of daily medication and documentation using the electronic health record. In addition, LPNs provide technical skilled nursing care and other health related duties to patients under the supervision of a Registered Nurse.

Description:

  • Provide healthcare to patients inside the facility.
  • Collect data for assessment of care needs, report findings and deliver appropriate nursing interventions under the direction of a Registered Nurse or Health Care provider.
  • Monitor Patients' progress and report outcomes appropriately.
  • Administer medication and complete related pharmacological procedures.
  • Complete and process all required documentation in a comprehensive and objective manner.
  • Use multilingual capabilities for accurate oral translation and communication to ensure continuity of care; Use "language line" translation service to ensure continuity of care.
  • Identify and fulfill educational needs to enhance nursing skills.
  • Complete required classes and education credits.
  • Maintain current BLS (CPR) certification.
Knowledge, skills, and abilities:
  • Exhibit a professional image in both appearance and behavior and maintain professional boundaries with patients.
  • Must be willing to work various assignments and shifts as needed.
  • Must be willing to float to various facilities as needed.
  • Demonstrate knowledge and compliance with applicable safety policies and OSHA standards.
  • Must be able to respond to a man down (emergency situation, must maintain current Cardiopulmonary resuscitation (CPR) certificate, and perform CPR.
  • Use multilingual capabilities for accurate oral translation and communication to ensure continuity of care; Use "language line" translation service to ensure continuity of care.

    Qualifications:

    • Completion of Licensed Practical Nurse program and avalid unrestricted license as a Licensed Practical Nurse issued by the Arizona State Board of Nursing (or issued by another state recognized by the Arizona Board of Nursing as a multi-compact license state) with no current, pending, or prior disciplinary action on nursing license.
    • New Grads Welcome
    • BLS/CPR/First Aid certification training is provided during new hire orientation.
    • Must pass fingerprint/background check.
    • Must be able to respond to a man down (emergency situation).
    • Must be able to get on floor to perform CPR chest compressions.
    • Must be able to walk up to 4 miles in a 12 hours shift.

      Preferred education and/or experience:

      • Two (2) or more years of experience as a LPN.
      • Experience in medical setting requiring large medication passes.
      • Bilingual (English/Spanish) skills.
      • Preferred Intravenous Infusion therapy/Venipuncture certification.
