Seeking a BE/BC Internal Medicine Physician to join dedicated team to accommodate growth and additional access to health care for our 11,000 patients in northwest NH.

As a Patient-Centered Medical Home (PCMH), we prioritize outcomes and integrated care, providing primary care, behavioral health, and chronic disease management to almost 10,000 patients across 26 towns in southern Coos County and northern Grafton counties.

In this role you will enjoy:

• Group currently has 8 Family Practice Physicians, 7 APRNs, 4 PAs, 1 Ped, 1 Internal Med Physician, 1 Pharmacist, 3 Dentists, 5 Behavioral Health specialists attended by a robust team of clinical and non-clinical support personnel.
• A balanced focus on quality & volume allowing more time with patients: average 20+ min visits.
• IT support team with a strong, clinical knowledge with >20 years of EHR experience.
• Integrated clinical pharmacy, dental and oral health services, behavioral health and patient navigator support, nutrition and paralegal services.
• Emphasis on self-care for patients and staff: provider driven autonomy in practice.
• Varied patient mix – pre-natal through the end of life.
