Our client hospital serves a close-knit island and surrounding communities through a 25-bed critical access facility in Bar Harbor and a network of area health centers—all designed to provide comprehensive healthcare for residents and visitors.

The Hospital received a five-star rating, the highest awarded, from the Centers for Medicare and Medicaid Services in 2020. In addition, the organization’s inpatient, outpatient, emergency and surgical care services have been consistently recognized by Avatar International, the Leapfrog Group, the National Rural Health Association, Harvard Pilgrim and Healthgrades.

They have an immediate need for an Interim Director of Case Management.

UR is handled by another department, this will be mostly discharge planning in and outpatient.

The Director of Care Management participates in major programming for holistic care throughout the community; serves as the leader for the activities of the care management and social work staff; oversees the coordinator of the federally designated diabetes program and serves as a leader for primary care activities in collaboration with senior management, medical staff and community participants. Other areas of responsibility include: Coag clinics, Community Care Team and Cardiac Rehab department.
