Interim Case Manager - CLOSED
This position is responsible for assessment, counseling and social services for patients within
the behavioral healthcare setting.
Responsibilities
-Provides assessments, treatments and case management to patients and
families.
-Plans and coordinates therapeutic program services on the behavioral health
unit.
-Facilitates individual and group therapy sessions with patients to
establish/review goals and document progress toward goals.
-Participates in discharge planning, arranges for follow up care and develops
mechanisms for exchange of appropriate information with sources outside the
hospital.
-Completes psychosocial assessments on all new admissions within CMS
guidelines.
-Develops the master treatment plan in a timely manner according to program
policies and documents patient status and response to treatment throughout
hospitalization.
-Leads the scheduled treatment team meetings.
-Acts as the unit liaison including coordination with the assigned individual
evaluator for involuntary commitments.
-Acts as the unit representative during any court proceedings related to
involuntary commitments.
-Serves as a resource person to healthcare professionals concerning psychosocial
problems and community resources.
-Educates patient and families on signs and symptoms of illness to prevent rehospitalization
and improve disease management skills.
-Maintains performance improvement activities within the department and
participates in performance improvement activities as assigned.
-Performs other duties as assigned or requested.
Qualifications
-Master’s degree in social work (MSW) required.
-Current licensure as a Licensed Clinical Social Worker or Licensed Marriage and Family
Therapist, preferred
-Two or more years of previous experience in all aspects of social service with an
emphasis in psychiatric work.
-Current certification/training in the management of disruptive and assaultive behavior.
-BLS
-CPI training will be provided
-Basic computer knowledge
-Strong interpersonal skills
