Linke Resources is seeking a Intensive Preventive Services (IPS) Case Manager on behalf of our client, to join a team dedicated to improving the lives of people in the community. As a IPS Case Manager, you will promote pro-social behavior for at-risk youth by providing a comprehensive set of services.

Location: Philadelphia, PA

Responsibilities:

• Conduct home visits on a bi-weekly basis.
• Conduct regular school visits as well as collateral agency visits as needed.
• Develop and monitor academic, mentoring plan in conjunction with input from youth, school faculty, and others.
• Provide support and case management services to families and individuals
• Accompany youth and/or families to appointments as needed.
• Maintained thorough case files, including data entry, case notes, and all required documentation.

Requirements:

• Bachelor's degree in a human services field (e.g., psychology, education, sociology, and criminal justice.)
