Hospital Registered Nurse Care Manager

We are seeking a dedicated and experienced Hospital Registered Nurse Care Manager to join our team. This pivotal role focuses on managing patient care from initial assessment upon admission to discharge planning, ensuring a seamless transition and continuity of care. This position requires a proficient caregiver with a proactive approach, capable of assessing patient needs and managing comprehensive care plans effectively. Salary: DOE per year

Essential Duties and Responsibilities:

  • Perform initial comprehensive assessment upon patient admission to determine immediate care requirements.
  • Provide ongoing patient assessments to identify changing needs and ensure that care plans are current and effective.
  • Monitor the effectiveness of all ongoing services to guarantee that they meet patient needs and clinical standards.
  • Track and document progress toward individual healthcare goals to ensure optimal outcomes.
  • Oversee patient transitions within the hospital and to subsequent care settings, effectively coordinating all discharge planning activities.

Minimum Position Qualifications:

  • Bachelor of Science in Nursing (BSN) degree required.
  • Active New York RN license/registration is mandatory.
  • At least one year of Hospital Care Management experience.
  • Current certification in Care Management.
  • PRI Certification required.

Preferred Qualifications:

  • Advanced problem-solving skills and the ability to work effectively under pressure.
  • Excellent organizational skills and the ability to handle multiple priorities simultaneously.
  • Strong communication and interpersonal skills to effectively collaborate with medical teams and communicate with patients and their families.
  • Experience with electronic health record (EHR) systems.
