We are seeking a dedicated and experienced Hospital Registered Nurse Care Manager to join our team. This pivotal role focuses on managing patient care from initial assessment upon admission to discharge planning, ensuring a seamless transition and continuity of care. This position requires a proficient caregiver with a proactive approach, capable of assessing patient needs and managing comprehensive care plans effectively. Salary: DOE per year
Essential Duties and Responsibilities:
- Perform initial comprehensive assessment upon patient admission to determine immediate care requirements.
- Provide ongoing patient assessments to identify changing needs and ensure that care plans are current and effective.
- Monitor the effectiveness of all ongoing services to guarantee that they meet patient needs and clinical standards.
- Track and document progress toward individual healthcare goals to ensure optimal outcomes.
- Oversee patient transitions within the hospital and to subsequent care settings, effectively coordinating all discharge planning activities.
Minimum Position Qualifications:
- Bachelor of Science in Nursing (BSN) degree required.
- Active New York RN license/registration is mandatory.
- At least one year of Hospital Care Management experience.
- Current certification in Care Management.
- PRI Certification required.
Preferred Qualifications:
- Advanced problem-solving skills and the ability to work effectively under pressure.
- Excellent organizational skills and the ability to handle multiple priorities simultaneously.
- Strong communication and interpersonal skills to effectively collaborate with medical teams and communicate with patients and their families.
- Experience with electronic health record (EHR) systems.
- Knowledge of healthcare regulations and accreditation standards.
