Hospital Patient Case Assistant

Our client, a welll known Hospital System in NY is looking for a Patient Case Assistant. In this role you will work with the hospital team to coordinate the logistics for patients that are being discharged from the hospital. You will Work with…

• hospital team to facilitate execution of the discharge plan
• internal staff (doctors, nursing, social work) to assure the discharge plan is clearly defined and understood
• patients to explain status of various parts of discharge plan (Placement), and needs
• patient family to assure understanding of status
• external organizations (nursing homes) to assure there is a place for patient and an agreed upon date and time of placement
• external organizations (DME) to assure medical equipment is available at date and time needed

• 40 hour work week
  • Possibly 4-10 hour shifts

  • On site

• Requirements:
  
  • Staff level/ administrative level positions
  • HS Diploma
  • Patient interaction experience would be a good fit (access, registration, discharge planning)
  • Medical facility (IP/ outpatient) registration, scheduling
  • Social Service (clerical, assistant) agency type experience would be a fit
  • Great communication and persuasion skills
  • Detail oriented
  • Relentless commitment to patient discharge
