Essential Duties and Responsibilities:

• Perform thorough physical evaluations and consider medical histories to accurately assess patient conditions.
• Develop individualized treatment plans focusing on achieving the specific goals set for each patient.
• Administer manual exercises, therapeutic techniques, and other specialized treatments to enhance physical functions.
• Advance patient independence by educating patients and their family members on proactive wellness and injury prevention.
• Meticulously record patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
• Collaborate closely with other healthcare professionals to deliver holistic care tailored to home health contexts.

Minimum Position Qualifications:

• Must have completed a Physical Therapy Program accredited by a recognized body.
• Requires a minimum of nine months working experience as a Physical Therapist.
• Demonstrated ability to effectively communicate and empathize with patients, providing compassionate care.

Preferred Qualifications:

• Experience in home health care settings is highly advantageous.
• Advanced certifications related to physical therapy in fields such as neurology or orthopedics are desirable.
