Primary focus: Behavior change, education, SDOH Navigation, engagement
- Motivational interviewing; action plans for diet/activity/smoking cessation/sleep; end tailored materials.
- SDOH screening and resource linkage (Transport, food, utilities); appointment coordination; group classes
- Device/app onboarding (portals, trackers) PROs Collection (CAT, PROMIS, Etc.)
- Reinforce care plan; escalate symptoms/med questions to LPN/RN; document and log CCM minutes appropriately (per payer rules)
- Typical direct care time 70-80% of FTE.
