Full-time Physical Therapist - Home Health
We are seeking a dedicated and experienced Physical Therapist to provide comprehensive therapeutic services in a home health setting. In this role, you will manage a patient load, delivering high-quality, individualized care directly in patients' homes, supporting the continuity of care. The ideal candidate will have expertise in evaluating and adjusting care plans, proficient communication, and the ability to foster positive outcomes by educating patients and families on ongoing management and exercises.
Salary: $95,000.00 - $112,000.00 per year
Essential Duties and Responsibilities:
- Conduct thorough patient evaluations to develop effective, patient-centered care plans.
- Provide skilled physical therapy services under the direction of a prescribed physician's plan of care.
- Educate and train patients, families, or caregivers in exercises, mobility, and therapeutic practices necessary to improve recovery.
- Assess, monitor, and document patient progress within established timelines and adherence to care standards.
- Coordinate and prioritize patient caseload effectively, ensuring timely handoffs and staffing management is maintained.
- Utilize evidence-based practices and clinical skills to promote optimum physical functioning and independence of patients.
Minimum Position Qualifications:
- Licensed as a Physical Therapist in the applicable State.
- A current unencumbered State professional license.
- Valid driver's license, reliable transportation, and proof of insurance necessary for home visits.
- One year of experience in home health care or related field, demonstrating a comprehensive understanding of the OASIS-C assessment process.
- Strong ability to make professional clinical judgments and adapt treatment plans accordingly.
Preferred Qualifications:
- Advanced certification or specialized training in physical therapy for home health care.
- Experience with electronic documentation and clinical software systems.
- Demonstrated leadership capabilities in managing caseloads and directing PTAs or other support staff.
- Strong communication skills with a proficiency in English, both verbal and written.
Skills:
- Exceptional clinical skills with the ability to perform comprehensive assessments.
- Proficiency in utilizing physical therapy appliances and in teaching about their use.
- Excellent organizational and time management skills.
- Ability to work independently in a home health setting.
- Strong computer skills, including proficiency in email, clinical, word processing, and spreadsheet software.
