Full-time Physical Therapist - Home Health

We are seeking a dedicated Full-time Physical Therapist to join our dynamic home health team. In this role, you will provide direct patient care to homebound patients to alleviate pain and foster health improvements, following physician-set treatment plans. You will play a crucial role in improving patients' quality of life, enabling them to regain bodily functions and achieve personal health goals while remaining in their homes. This role requires a blend of expertise in physical therapeutic interventions, patient and family education, and a profound commitment to enhancing patient outcomes.

Salary: $95,000.00 - $122,000.00 per year

Essential Duties and Responsibilities:

  • Provides treatments as ordered by the attending physician to help patients regain or enhance their physical capabilities.
  • Meticulously documents treatments and findings in the electronic medical records.
  • Notifies attending physician and other healthcare team members promptly about significant changes in the patient's condition.
  • Promotes patient and family education using tailored communication techniques that consider cultural, ethnic, or personal preferences.
  • Empowers patients and their families by teaching appropriate self-care techniques.
  • Ensures timely submission of accurate documentation in alignment with Agency policies and regulatory requirements.
  • Actively participates in agency-performed activities such as in-services, Quality Assurance and Performance Improvement (QAPI), and team meetings.

Minimum Position Qualifications:

  • Current unencumbered State professional license as a Physical Therapist.
  • Ability to maintain and handle confidential information professionally and ethically.
  • Excellent verbal and written communication skills, with fluency in English.
  • Proficiency in using personal computers for clinical, word processing, and spreadsheet software.
  • Valid driver's license, reliable automobile for work use, good driving record, and proof of auto insurance.

Preferred Qualifications:

  • At least one year of experience as a home care professional physical therapist.
  • Competency in performing comprehensive assessments and OASIS-C documentation.
  • Proven proficiency in clinical skills relevant to home health care.
  • Knowledge of current federal, state, and agency regulations relating to patient care, including incident management and reporting.
