Location: Union, NJ
SNF/LTC Group with 9 buiildings in the Northern New Jersey area is seeking experienced an MDS Coordinator to support all MDS activities in the facilities. This position is responsible for supporting the development and completion of the Resident Assessment Instrument (MDS) in accordance with state/federal requirements and the policies/goals of the facility. This person is a key participant of the interdisciplinary team, assisting in ensuring quality of care, continuity of services, and individualized patient-centered plans and goals.

Responsibilities
• Travel between the facilities as needed. All buildings are within an hour and a half of each other.
• Training/education of MDS staff in facilities as needed.
• Conducts patient assessments and determines the health status, level of care, an any subsequent changes.
• Maintenance of Operational Documentation
• Be familiar with current MSDS
• Ensures accurate and timely RAI completion.
• Coordinates interdisciplinary participation in completion of the RAI and resident care plan.
• Assists in developing and implementing programs and procedures to maximize the reimbursement potential of the facility and improve quality of care.

Qualifications
• Ability to work full time (40 hours)
• Current active RN NJ licensure
• 3 years clinical experience and at least 2 in a LTC setting
• One year of management/supervisory experience is preferred
• MDS Coordinator and RUG category exposure/experience
• Knowledge of Medicare/Medicaid regulations and benefit guidelines
