Enrollment Case Manager
Hire Professionals is seeking a dedicated, detail-oriented, and community-focused Enrollment Coordinator to support one of our valued clients. In this role, you will travel throughout your assigned county to conduct in-home intake visits for waiver applicants and facilitate the eligibility determination process.
This position is essential in helping individuals access important community-based services, requiring strong interpersonal skills, thorough documentation, and a commitment to supporting vulnerable populations.
Summary of Qualifications
- Travel throughout assigned county to conduct in-home intake visits for waiver applicants.
- Facilitate the eligibility determination process from initial identification to full program enrollment.
- Provide assistance to applicants throughout the entire enrollment journey—from first contact to confirmed participation.
- Review and screen provider applications and accompanying documentation for completeness and accuracy.
- Initiate and assist in the development of the individual service plan, ensuring proper approvals are obtained.
- Develop and maintain relationships with community-based organizations, advocates, and stakeholders to gather feedback and identify opportunities to improve the application and enrollment experience.
- Contact providers to obtain missing or incomplete information related to applications.
- Collaborate with Site Visit Field Staff to collect, verify, and report site visit findings.
- Maintain accurate, timely, and compliant documentation for all cases handled.
Requirements
- Bachelor’s Degree required.
- Minimum of one year of experience in public or private social work.
- Minimum of two years of casework experience, including at least one year assessing clients’ functional abilities to determine eligibility for institutional or community-based services.
- Strong communication, documentation, and organizational skills.
- Ability to work independently, manage travel schedules, and perform field-based responsibilities.
