Director of Patient Safety

Currently seeking a talented and dedicated Director of Patient Safety for a great opportunity in the Northeastern, TX area. The ideal candidate will possess 5+ years of current experience working in an acute care hospital and have 3+ years of experience working in a Patient Safety Hospital, Ambulatory Surgery Center in a leadership role.


Responsibilities of the Director of Patient Safety

  • Coordinates Patient Safety programs for the facility. Responsible for promoting safe and efficient work environments for all staff members and patients
  • Coordinates facility wide risk and patient safety related programs for the continuous improvement of the quality of care and service affecting all customers - patients, physicians, employees and payors
  • Effectively report, investigate, and analyze patient safety incidents, medical errors and potential risks in the facility. - Facilitate thorough and credible serious event analysis that result in strong sustainable improvement strategies
  • Facilitate thorough and credible failure mode effect analysis to identify and mitigate unintended adverse patient outcomes and evaluate effectiveness of process changes
  • Perform Patient Safety Rounds that identify patient safety risks
  • Empower staff to identify and participate in resolution of patient safety concerns
  • Coordinate disclosure of serious events to patients and/or families in accordance with organizational policy and regulations
  • Assure timely reporting of Patient Safety Work Product (PSWP) to the Patient Safety Organization
  • Actively participate in PSO learning collaboratives


Requirements of the Director of Patient Safety

  • 3 years' experience in Patient Safety management leadership required
  • 5 years' experience in Patient Safety required
  • BSN is required
  • Master’s Degree in Public Health/Nursing/related field Preferred
  • Certification in Patient Safety (CPPS) required; If not already CPPS, must obtain certification within 12 months of hire
  • Other licenses/certifications required by State
  • Team STEPPS Master Trainer. If not already a Master Trainer must obtain with 12 months of hire
