Position Responsibilities:
  • Lead and coach an interdisciplinary clinical team to deliver consistent, high-quality hospice care aligned with regulatory requirements and Kara Health standards
  • Maintain visible field leadership through case reviews, huddles, ride-alongs, and performance coaching
  • Oversee staffing, scheduling, territory coverage, and resource allocation to ensure daily patient coverage and efficient utilization
  • Monitor productivity, caseload distribution, visit volumes, and payroll accuracy to support operational efficiency and care continuity
  • Operate using a metrics-driven approach, reviewing performance data, identifying trends, and leading corrective action plans
  • Ensure full compliance with CMS hospice Conditions of Participation, state regulations, accreditation standards, and audit readiness
  • Lead quality initiatives, including QAPI, documentation accuracy, infection control, medication safety, and patient safety
  • Partner with Medical Director, Executive Director, and interdisciplinary teams to align care plans with patient goals and best practices
  • Uphold joint venture performance expectations and collaborate with health system partners on quality and care integration
  • Maintain strong communication and working relationships with internal teams, facilities, physicians, and community stakeholders
Experience & Skills:
Required Experience and Qualifications:
  • Active Registered Nurse (RN) license in state(s) of operation; BSN required, MSN preferred.
  • Minimum 5–7 years of hospice or home health leadership experience, with at least 3 years in a Director-level or senior management role overseeing a multidisciplinary team.
  • Proven success managing teams of 20+ clinical staff and patient populations exceeding 100 active census.
  • Demonstrated ability to use EMR systems and analytics tools to drive data-based performance improvement.
  • Strong knowledge of hospice regulations, quality reporting, and compliance frameworks.
  • Excellent communication, leadership, and conflict-resolution skills.
Compensation $175k-$180k, Health Insurance, 401k, Paid Vacation
