Currently seeking a talented and dedicated Case Management Director for a great opportunity in the Southern TX area. The ideal candidate will possess 5+ years of current experience working in Case Management in an acute care hospital and have 3+ years of experience working in a Case Management unit in a leadership role.
Responsibilities of the Director of Case Management
- The Director of Case Management (DCM) provides hospital leadership for operations and management of the case management staff and department
- Develops and mentors the case management staff and is accountable for evaluating staffing levels and budget management
- Will execute, monitor, and evaluate the system and facility initiatives
- Ensures staff competencies in all areas of case management (utilization review, resource management, and transition/discharge planning, and social work services)
- Develop strong relations with hospital senior management, key medical staff, CNO/nursing leadership, HIM/CDI leadership to facilitate quality and efficacious care
- Leads hospital and system-wide case management initiatives to meet established benchmarks, standardization, and established targets
- Collaborate with Performance Improvement on CMS clinical indicators to ensure best practice is provided on behalf of our patients
- Demonstrates Services Excellence at all times
- Other duties as assigned
Requirements of the Director of Case Management
- Graduate from an accredited School of Nursing
- Bachelor’s degree required, Master’s preferred
- Current RN License
- Knowledge of managed care and capitation
- 5+ years of case management and discharge planning experience in an acute hospital setting
- 3+ years in a Director/Manager role over Case Management/Utilization Review
- Knowledge of managed care and capitation
