Out client is a state of the art, 10 year old community hospital with 150 beds and a reputation for excellence. This facility is seeking an AMAZING Director of Case Management to join their team! There are many reasons to consider working for this hospital and since their inception, they have received awards that are too numerous to mention but some of the highlight include:
  • STEMI Receiving Center designation
  • Stroke Receiving Center designation
  • Advanced Primary Stroke Center accreditation by The Joint Commission
What's even more important, Patients are treated as an integral part of the care team, encouraged to collaborate with doctors and nurses in making decisions that affect their care. Families are invited to participate in the process.

The ideal candidate would include:

EDUCATION/TRAINING/ EXPERIENCE:
  • Bachelor's degree from an accredited College or University in related field required.
  • Master's degree from an accredited College or University in related field preferred.
  • Five (5) to Seven (7) years of clinical experience in Case Management/Utilization Review required.
  • Minimum three (3) years leadership experience in Case Management/Utilization Review required, acute care experience preferred.
CERTIFICATIONS/LICENSES:
  • Current license as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) in the State of California.
  • Current Basic Life Support (BLS) or CPR Certification through the American Heart Association. Must be obtained within 90 days of start date.
  • CCM or ACM certification preferred
