Nealy Pierce has partnered with a growing, independently owned multi campus non-profit hospital in the Southern Gulf Region to recruit a dynamic and inspirational Director of Care Coordination to join their leadership team. More details follow below:

About the Hospital and the Director of Care Coordination Position:

  • Non-profit, award winning 400+ bed acute care hospital that has served its community for over 70 years and a member of a regional healthcare system
  • The Director will manage a team of 45 FTE’s including a Director and three Team Leaders and will be responsible for the hospital wide Care Coordination Team including Case Management, Utilization Review, Denial Review, and Social Work Services
  • Reports to a visionary Chief Medical Officer who has been with the organization for over 10 years

Director of Care Coordination Requirements:

  • Bachelor's degree required in Nursing (BSN) or Master’s Degree in Social Work required
  • Active RN or LCSW required
  • Minimum five (5) years of progressive Care Coordination leadership experience in an acute care hospital required
  • Working knowledge of Information Systems Management
  • Seeking a RELATIONSHIP DRIVEN, VISIBLE and INSPIRATIONAL leader
  • Proven track record of success in Care Coordination leadership
  • Able to establish positive collaborative relationships across all levels
