Clinical Counselor/ Therapist- Floater

Perform individual and group counseling and case management of assigned behavioral health patients to ensure coordination of services, in accordance with established treatment plan.


  • Provide individual and group substance abuse counseling
  • Serve as primary contact/clinical liaison to patients, patient’s families and community to ensure delivery of effective and appropriate treatment, in accordance with established program
  • Provide community outreach and network with other agencies and community resources
  • Review charts of assigned patients to determine appropriate referral services, based on recommended treatment plan
  • Facilitate, schedule and/or participate in clinical team meetings to monitor patient progress against goals; update progress notes and patient chart of all patient contacts, within established timeframe
  • Collaborate with patient, family members and other involved parties to assess barriers to attainment of goals and/or resources
  • Respond to emergency and/or crisis situations; contact appropriate individuals and/or agencies to ensure appropriate intervention and actions to optimize patient safety
  • Work closely and cooperatively with other programs, acting as point of contact for communication/coordination of care
  • Manage and oversee assigned case load to ensure delivery of services identified for continuity of care
  • Develop and implement transition, discharge and aftercare plans of assigned patients
  • Perform Behavioral Health assessment of patient and develop treatment/service plan to meet determined care needs; revise care plan as appropriate
  • Investigate and follow up on patient complaints to ensure timely response and/or appropriate resolution to concerns/issues
  • Identify appropriate community resources and perform follow up as appropriate
  • Conduct patient visits to monitor progress and determine suitability of environment for successful treatment outcomes
  • Work closely and cooperatively with peers to review and/or revise standards, policy and procedure manuals based on findings and/or recommendations
  • Participate in departmental orientation, on the job training and quality assurance programs/initiatives
  • Participate in a variety of department and hospital educational programs to maintain current skill and competency levels; identify and discuss performance or training needs with Supervisor


  • Master’s degree in Social Work or closely related field; equivalent combination of education and experience
  • Current State of Arizona license from the Arizona Board of Behavioral Health Examiners i.e. (LPC, LAC, LCSW, LMSW, LISAC, LASAC, LMFT, LAMFT)
  • CPR certificate within 30 days of hire
  • Must have at least 3 years previous experience
  • Must be willing to float between locations (Company vehicle will be supplied for travel)
