Community Health NP
Salem, OR area
M-F (8 hour shifts)
Salary is between $125,000-140,000 (DOE)
Telehealth and In-Person Visits (30 mile radius from your home)

Providing an extensive range of comprehensive care services for patients both well and ill residing in the community. The NP shall demonstrate a high level of clinical
expertise in working with individuals with acute and chronic conditions and is responsible for assessment, diagnosis, treatment, plan, management, education, health promotion and care coordination for member health needs.

Requirements

  • Graduation from an accredited school of nursing
  • Current license as a NP in the State of Oregon
  • Population health management with 1 year or more experience as Advanced Practice Nurse
  • Experience working with vulnerable adult populations
