Our client is seeking a successful Clinical Pharmacy Professional take a great unit to new heights of patient care.
The Clinical Pharmacy Specialist (CPS) is responsible for coordinating the clinical, teaching and research initiatives that support the Section of Clinical Pharmacology, Department of Pharmaceutical Care Services. The Clinical Pharmacy Specialist is responsible for assuring that optimal clinical services as offered under the auspices of the Department of Pharmaceutical Care Services are provided to patients on a daily basis. The Clinical Pharmacy Specialist facilitates the development, implementation, and maintenance of systems/programs that promote desirable patient outcomes through the use of appropriate, safe, and cost effective therapies, as well as promoting education and understanding of the same throughout the healthcare system.
The Clinical Pharmacy Specialist will serve as a clinical liaison between the Department of Medicine, Nursing and Pharmacy. This responsibility includes recommending appropriate, safe, and effective pharmacologic therapies for patients when consulted, developing associated protocols as needed, and participating in other patient-care activities. Drug information related to the aforementioned will be provided to medical, nursing, and pharmacy staff as appropriate. The CPS provides direct clinical support of the attending pharmacist's unit activities. In addition, the Clinical Pharmacy Specialist will cross-over clinical services (including but not limited to antimicrobial stewardship, anticoagulation clinic, internal medicine (intensive pharmacy co therapeutics) and pain/palliative care service) when necessary. Lastly, the Clinical Pharmacy Specialist acts as an active member of various committees, subcommittees, and workgroups as appointed.
The Clinical Pharmacy Specialist is expected to dedicate 70% time-effort to Clinical Practice (as above), including precepting and teaching residents, students and other healthcare professionals alike. 15% time-effort is expected to maintain the administrative portion of committee services and inpatient staffing as requested. Additionally, 15% time-effort will be dedicated to the combined efforts of Scholarship (including but not limited to research and publication).
Education/Qualifications:
- Doctor of Pharmacy Degree from an ACPE-Accredited School of Pharmacy
Experience:
- Post-Doctoral Residency and/or Fellowship training in Clinical Pharmacy Practice and/or one of its sub-specialties (Pain/Palliative Care, Infectious Disease, Internal Medicine, etc.)
Licensure/Certificates:
- NY State Pharmacy license or eligibility for with intent of obtaining upon acceptance of position
- Conduct of appropriate amounts of continuing education to maintain licensure in NYS
- Certification in (or intent to obtain upon acceptance of position) and pharmaceutical sub-specialties (BCPS, BCOP, CACP, etc.)
