Clinical Care Manager-

Registered Nurse, permanent part time role based in Sydney Northern suburbs

An AHPRA Registered Nurse is required to coordinate and delivery client care plans, ongoing management services to clients in their homes. Working in both the office and field environment the role will lead, direct and guide both carers and clients ensuring quality services. Experience in Aged Care, Disabilities and/ or community based experienced is required.

Experience Required

• Assessment and development of detailed and tailored client care plan
• Aged Care and/or disability experience
• Team Co-ordination skills
• Strong computer skills and Office and Administration experience
• Excellent communication skills and presentation
• High attention to detail
• Well-developed phone communication and customer service skills
• Influencing and negotiation skills
• Positive and mature attitude
• Well-spoken and detail-oriented
• Can manage a fast-paced changing environment.
• Displays natural empathy and is a natural people- person
• Experience within the in-home care industry
