Case Manager
We have exciting opportunities for compassionate, community-focused Case Managers to support individuals and families throughout the assigned county. This role is ideal for professionals who are passionate about helping others access critical services, guiding applicants through the eligibility process, and making a meaningful impact within the community.
As a Case Manager, you will travel throughout the assigned county conducting intake visits in applicants’ homes and assisting individuals through the waiver enrollment and eligibility determination process. This position requires strong communication skills, professionalism, empathy, and the ability to manage multiple cases effectively.
Position Summary
The Case Manager is responsible for conducting intake assessments, coordinating services, supporting waiver applicants, and facilitating the eligibility determination process for community-based programs. This role works closely with applicants, providers, community organizations, and internal teams to ensure a smooth and supportive enrollment experience.
Key Responsibilities
- Travel throughout the assigned county conducting intake visits for waiver applicants in their residences
- Facilitate the eligibility determination process for individuals seeking services and support programs
- Provide guidance and assistance from the initial identification of potential participants through successful program enrollment
- Review and screen provider applications and supporting documentation for accuracy and completeness
- Initiate development of Individual Service Plans (ISPs) and obtain required departmental approvals
- Assist in building relationships with community-based organizations, advocates, and stakeholders to improve access to services and reduce enrollment barriers
- Contact providers to obtain missing, incomplete, or additional required information related to applications
- Collaborate with Site Visit Field Staff to collect, review, and verify site visit documentation and findings
- Maintain accurate case records and ensure compliance with program policies and procedures
- Support individuals and families with professionalism, compassion, and strong communication throughout the process
Qualifications
- Bachelor’s Degree required
- Minimum of one year of experience in public or private social work required
- Two years of casework experience preferred
- At least one year of experience performing client assessments to determine the need for institutional or community-based services preferred
- Strong organizational, documentation, and communication skills
- Ability to work independently while managing multiple cases and deadlines
- Comfortable traveling throughout the assigned county and conducting in-home visits
- Knowledge of waiver programs, case management practices, and community resources preferred
