Our client is a rapidly growing home health company that’s become a national leader in providing affordable, innovative healthcare for Dual-eligible individuals.

They have many openings for Case Managers that are Bi-Lingual located in the NY Boroughs.

They have various Nurse Care Manager openings working in collaboration with the prospect, enrollment and sales to conduct clinical assessments, recommend appropriate level of services and create an individual care plans. The development, Individual Care Plans based on a member’s goals, preferences and disease states.  The goal of the individual care plan is to link members to appropriate care, services, and community resources, promote self-management in order to improve the health status, functioning, community integration and quality of life for all members in their preferred place of residence. 

Requirements:

·        Must be Bilingual (Spanish OR Chinese OR Cantonese OR Bengali)

·        UAS Assessment tool experience required

·        RN License in the State of NY

·        2+ year experience in Home Health or related field

·        Knowledge of Medicare and Medicaid

·        Up To 90% travel to client homes
