Advanced Practice Nurse - Skilled Nursing Facility

We are excited to offer a unique opportunity for a dedicated Advanced Practice Nurse to join our team within a hospital-owned Skilled Nursing Facility in the scenic Finger Lakes Region of NY. The role entails delivering high-level medical care to adults in a supportive and state-of-the-art environment. Salary: 120k to 135k per year

Essential Duties and Responsibilities:

  • Rounding on patients in a skilled nursing facility to assess and manage patient care.
  • Developing individualized care plans and coordinating with interdisciplinary teams.
  • Monitoring patient progress and adjusting treatment plans as necessary.
  • Providing leadership and guidance to nursing staff to ensure the highest care standards are upheld.
  • Participating in quality improvement initiatives to further enhance patient outcomes and facility operations.
  • Contributing to the research and analysis of patient data to improve practice and patient care.

Minimum Position Qualifications:

  • Certified as a Nurse Practitioner with current certification in Adult Medicine or Family Medicine.
  • Valid and active Nurse Practitioner license in the state of New York (NYS license).
  • Minimum of 1 year of experience working as a Nurse Practitioner.
  • Strong clinical skills with experience in a skilled nursing or related healthcare facility.
  • Ability to manage complex patient cases and multitask in a dynamic environment.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and staff.

Preferred Qualifications:

  • Advanced training or specialization in geriatric care or chronic disease management.
  • Prior experience in a leadership or supervisory role within a healthcare setting.
  • Proven track record of improving patient care and workflow efficiency.
  • Experience with electronic medical record (EMR) systems.
  • Commitment to continuous professional development and education.
  • Familiarity with regulatory compliance and healthcare accreditation standards.
